General Principles and Management of Acute Poisoning and Heavy Metal Poisoning: Notes and MCQs for Pharmacy Competitive Examinations
Acute poisoning is a common medical emergency in which rapid recognition and appropriate management can significantly reduce morbidity and mortality. Poisoning may occur due to accidental or intentional exposure to medicines, chemicals, pesticides, household products, industrial substances, heavy metals and other toxic agents. For pharmacy students and candidates preparing for competitive examinations such as UPSC Drug Inspector, GPAT, NIPER, AIIMS Pharmacist, Railway Pharmacist, SSC, ESIC and State Pharmacist examinations, a clear understanding of the general principles of poisoning management is essential.
The management of acute poisoning is not limited to identifying a poison and administering an antidote. The priority is stabilisation of the patient by following the fundamental principle of airway, breathing and circulation (ABC). Once the patient is stabilised, further management may include prevention of continued exposure, appropriate decontamination, reduction of poison absorption, supportive and symptomatic treatment, enhanced elimination of the toxic substance and administration of a specific antidote whenever indicated.
An important area of toxicology is heavy metal poisoning. Metals such as lead, arsenic, mercury, iron and copper can interfere with normal cellular and enzymatic functions and may produce serious systemic toxicity. Chelation therapy plays an important role in the management of selected heavy metal poisonings. Drugs such as dimercaprol, calcium disodium EDTA, succimer, deferoxamine and penicillamine are important agents frequently tested in pharmacy competitive examinations.
Students should also understand the rationale behind procedures such as gastric lavage, activated charcoal, urinary alkalinization and hemodialysis. Equally important is knowing when these procedures are contraindicated or ineffective. Many examination questions are designed to test concepts rather than simple memorisation, particularly the selection of an appropriate antidote, chelating agent or method of enhanced elimination.
