Chronopharmacology

Chronopharmacology is the study of how biological rhythms influence the effects, pharmacokinetics, and pharmacodynamics of drugs. It helps determine the right drug, right dose, and right time to obtain maximum therapeutic benefit with minimum adverse effects.

Biological Clock and Biological Rhythms

The biological clock is an internal timing system that regulates physiological and biochemical functions according to approximately 24-hour cycles. The suprachiasmatic nucleus (SCN) of the hypothalamus acts as the principal circadian pacemaker and is strongly influenced by the light–dark cycle.

Major types of biological rhythms include:

  • Circadian rhythm: approximately 24 hours; e.g., sleep–wake cycle and cortisol secretion.

  • Ultradian rhythm: shorter than 24 hours; e.g., pulsatile hormone secretion.

  • Infradian rhythm: longer than 24 hours; e.g., menstrual cycle.

  • Circannual rhythm: approximately one year; e.g., seasonal physiological changes.

Hormones, Diseases and Drugs

Hormones such as cortisol, melatonin, growth hormone, and insulin show rhythmic secretion. Several diseases are also influenced by circadian rhythms. For example, asthma symptoms may worsen at night, blood pressure generally follows a daily pattern, and gastric acid secretion varies during the day. Drug therapy can therefore be timed according to disease rhythms. Examples include evening administration of some antihypertensive and lipid-lowering drugs, and appropriately timed therapy for asthma and rheumatoid arthritis.

Chronokinetics

Chronokinetics refers to rhythmic variations in the absorption, distribution, metabolism, and excretion (ADME) of drugs. Changes in gastric emptying, hepatic enzyme activity, renal function, blood flow, and plasma protein binding may alter drug concentrations and duration of action at different times of day.

Chronotherapeutics and Future Scope

Chronotherapeutics involves administering medicines at a time when they are most effective and least toxic. It is particularly useful for diseases with predictable biological rhythms. Future developments may include time-controlled drug-delivery systems, programmable devices, personalised dosing schedules, and chronotherapy guided by biomarkers and wearable technologies.
