Introduction to Drug Interactions
Drug interactions are an important concept in pharmacology and clinical pharmacy because the effects of one drug can be altered when it is administered with another drug, food, beverage, or other substance. A drug interaction may increase or decrease the therapeutic effect of a medicine, alter its duration of action, or increase the risk of adverse effects. Understanding drug interactions is therefore essential for the safe and effective use of medicines, particularly in patients receiving multiple drugs.
Drug–drug interactions occur when two or more medicines influence each other’s pharmacological effects or alter the concentration of one another in the body. These interactions are broadly classified into pharmacokinetic and pharmacodynamic interactions.
Pharmacokinetic drug interactions occur when one drug affects the absorption, distribution, metabolism, or excretion of another drug. Consequently, the plasma concentration and exposure of the affected drug may change. For example, an antacid may reduce the absorption of certain drugs, enzyme-inducing drugs may increase the metabolism of other medicines, and some drugs may reduce renal excretion of another drug. Pharmacokinetic interactions can therefore modify the onset, intensity, and duration of drug action.
Pharmacodynamic drug interactions occur when drugs influence each other’s effects at their sites of action without necessarily changing their plasma concentrations. These interactions may be additive, synergistic, or antagonistic. For example, two drugs producing central nervous system depression may have an additive effect, whereas one drug may antagonise the therapeutic effect of another by acting on the same physiological pathway.
Drug interactions may be clinically insignificant in some cases, while others can result in treatment failure, toxicity, or serious adverse effects. The risk is particularly important in elderly patients and individuals receiving multiple medications. Knowledge of the mechanisms and clinical significance of drug interactions helps pharmacists and healthcare professionals identify potentially harmful combinations, optimise drug therapy, and improve patient safety.
