Factors Modifying Drug Action: Important Concepts for Competitive Exams

The response produced by a drug is not always the same in every individual. Several factors can modify the intensity, duration, and nature of drug action. These factors are highly important in pharmacology and are frequently tested in competitive examinations such as GPAT, NIPER, UPSC Drug Inspector, AIIMS Pharmacist, Railway Pharmacist, SSC, ESIC, and State Pharmacist examinations.

Important factors include age, body weight, sex, genetic variation, pathological conditions, route of administration, dosage, environmental factors, and previous exposure to drugs. Among these, tolerance, tachyphylaxis, idiosyncrasy, dependence, addiction, and combined drug effects are particularly important for examination preparation.

Tachyphylaxis refers to a rapidly developing decrease in response when a drug is administered repeatedly at short intervals. Ephedrine is a classic example, where repeated administration may deplete stored noradrenaline and reduce the response.

Tolerance develops when repeated or prolonged administration of a drug results in a decreased response, requiring a higher dose to produce the same effect. It may be pharmacokinetic or pharmacodynamic, and cross-tolerance can occur between pharmacologically related drugs.

Idiosyncrasy is an unusual and often genetically determined response to a drug that differs from the expected pharmacological effect. It should be distinguished from allergy and other adverse drug reactions.

Physical dependence is a state of physiological adaptation in which abrupt discontinuation of a drug may produce withdrawal symptoms. Addiction, in contrast, is characterised mainly by compulsive drug-seeking and continued use despite harmful consequences.

When two drugs are administered together, their effects may also change. In an additive effect, the combined response is approximately equal to the sum of their individual effects. In synergism, the combined effect is greater than the expected sum of the individual effects. These concepts are particularly important in understanding drug interactions and therapeutic combinations.
